Client Waiver, Informed Consent & Liability Release


1. Health Disclosure

I confirm that I have consulted a physician or believe I am able to participate in exercise safely. I have disclosed all relevant medical conditions, injuries, and medications to my coach and agree to update them if anything changes.


2. Assumption of Risk

I understand that physical training (including weightlifting, cardio, conditioning, mobility, and use of gym equipment) carries inherent risks such as muscle strain, joint injury, aggravation of existing conditions, and other unforeseen physical issues. I voluntarily assume all risks associated with participation.


3. Liability Release

In consideration of training with Onyx Strength & Conditioning, I release and discharge Onyx, its owners, coaches, contractors, and affiliates from any liability, claims, or demands for injury, illness, or damages arising from participation, except in cases of gross negligence or intentional misconduct. This release applies to me, my heirs, and assigns.


5 Optional Media Consent

I give permission for training photos/videos to be used for educational or promotional purposes.

Online Coaching / Training Agreement

Fitness Services


  • Exercise instruction and supervision

  • Strength and conditioning programming

  • Technique and performance coaching

  • General fitness education

Nutrition Services


  • Nutrition education and coaching

  • Habit-based nutrition guidance

  • Calorie and macronutrient targets

  • Lifestyle and behavior support related to nutrition

  • Customized meal plans and sample menus (paid services)

  • Food structure, portion guidance, and meal timing strategies

Participant Responsibilities


The Participant agrees to:

  • Provide accurate health, training, and nutrition information

  • Disclose medical conditions, medications, allergies, or dietary restrictions

  • Use discretion and personal judgment when following meal plans

  • Communicate adverse reactions or concerns immediately

Payment & Billing


  • All services, including meal plans, must be paid in advance

  • Fees are non-refundable unless otherwise stated in writing

  • Meal plans are considered a professional service and are non-transferable


 Cancellations & Missed Sessions


  • A minimum of 24 hours’ notice is required to cancel or reschedule training sessions

  • Missed sessions without notice may be forfeited

  • Late arrivals may result in shortened sessions without refund


 Assumption of Risk


The Participant understands that physical training and nutrition changes may involve risks, including:


  • Muscle soreness or injury

  • Digestive discomfort or food intolerance reactions

  • Changes in body weight, energy levels, or appetite


The Participant voluntarily assumes all risks associated with participation.


Release of Liability


The Participant releases and holds harmless Onyx Strength & Conditioning, its coaches, employees, and affiliates from any claims, injuries, or damages arising from fitness or nutrition services, including meal plans, except in cases of gross negligence.


Termination of Services


The Coach reserves the right to terminate services if:

  • The Participant engages in unsafe behavior

  • Payment obligations are not met

  • Communication becomes abusive or inappropriate


The Participant may discontinue services at any time, subject to payment terms.
